Vision Plan Details

 

Chambliss Bahner & Stophel offers employees a dual option vision plan through Guardian.

​

The Davis Vision network is made up primarily of retail vision providers such as Wal-Mart, JCPenney, Sears, & Sam's Club. This option offers more flexible hours to receive vision care.

• You'll be able to search via location and also services and other demographics that will help streamline your search.

The VSP network is the nation's largest vision network and is made up primarily of private practice vision providers.

• To find a provider in the VSP network, visit vsp.com, then click "Find a Doctor" at the top of the page.

• You can search via Location, Office or Doctor.

The plan features a $20 eye exam copay (one per every 12 months) and a $20 materials copay. You will be able to get frames every 24 months and lenses or contacts every 12 months.

Employees will pay the full cost of this coverage.
